insurance policyFor our patients with Dental Benefits:

Many of our patients have a dental benefit either through an insurance plan provided by their employer or through a privately purchased plan. Each policy is written differently with different coverage as well as exclusions. But each plan allocates a maximum dollar amount payable for specific dental services, determined by the carrier, up to a total allowable benefit per year.  Oftentimes, eligible benefits for treatment are dependent on the premium paid by the employer, or the individual, to the insurance company.

Colony Square Dental Associates (CSDA) is not contracted with any managed care plans; we are considered ‘out of network’. This means that if you have a plan that requires you to select an office from a list of providers, our dentists will not be on the list and benefits are not payable for care in our office. However, if your plan allows you to utilize your benefits at a dental office of your choosing (often termed a PPO plan), we are happy to assist you in claims submission. And we gladly provide the carrier with any needed dental x-rays and a written diagnostic report. Should your insurance company have any questions about the services provided we promptly follow up via phone or mail to help ensure you obtain the maximum benefits due. When a patient utilizes their PPO plans, out of network at CSDA, oftentimes the benefits are the same as going in network; however, with some plans the allowable benefit could be lower.

Most insurance companies respond to claims within 4-6 weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. We do ask that you remit all balances not settled by your insurance company within 60 days of care.

And for extensive treatment, when an investment in your oral health is necessary or you wish to enhance the aesthetics of your smile, we offer various payment plans, including CareCredit.
